DD Form 2946 Department of Defense Telework Agreement (updated 10/11/11)
Cost Analysis Form (this must be completed if duty station changes)
Situational Telework Agreement Spreadsheet

Outlook Web Access (OWA) on Home Computer (please contact the Pax River Telework Coordinator for this form)

Dependent Care - Telework is not to replace dependent care.  A telework agreement cannot be approved to supervise children (or adults) while an employee is teleworking.  A separate caregiver must be present in the home while an employee is teleworking.  The employee cannot provide care to a dependent while teleworking, they may take their lunch break to visit or attend to the dependent.

How to apply for Telework:

For Regular and Recurring Telework agreements, please complete the DD Form 2946 Department of Defense Telework Agreement in its entirety.  The employee signs the first, second and third page.  Obtain your supervisor's approval and signature.  The supervisor would sign the first page as the authorized management official and also the third page under supervisor's signature.

If the employee teleworks for more than half of their work-time at their telework site, and it is outside the locality pay area, a change of duty station will have to occur.  The employee will have to fill out the Cost Analysis Form and obtain Level 1 approval.

All Regular and Recurring Telework agreements must be sent to the Pax River Telework Coordinator for final approval before the employee may begin their telework.  An email will be sent to the employee (cc'd to their supervisor), letting them know their telework agreement has been approved or disapproved.

For Situational Telework agreements, please complete the DD Form 2946 Department of Defense Telework Agreement in its entirety.  The employee signs the first, second and third page.  Obtain your supervisor's approval and signature.  The supervisor would sign the first page as the authorized management official and also the third page under supervisor's signature.  The Situational Telework agreements do not have to go through the Pax River Telework Coordinator.  For tracking purposes, the Situational Telework Agreement Spreadsheet needs to be filled out and emailed to the Pax River Site Telework Coordinator.  Please call 301-342-6860 for the email address.
